VERMONT STATE HOSP - WATERBURY, VT

On or about 9/5/99 I was involuntarily committed to
the Waterbury State Hospital from CVH for mental evaluation and observations. My stay was short; only a day or two and then I was admitted to UVM Pyschiatric
for a few days.
I need a to get the paperwork of the evaluation and diagnosis for further treatment at a local health care provider and for further legal considerations.
Please send a release authorization form and discharge papers in order that I can sign them an have the informatio.
